It is a muscular pump. The detrusor muscle must squeeze urine through the prostate, and when it repeatedly pushes against resistance over years or decades, that muscle can remodel and become thicker. One number to remember: 2 millimeters. Research discussed in this episode has repeatedly examined DWT ≥2 mm as a potential signal of bladder outlet obstruction. Studies have reported sensitivity around 79–83% and specificity around 89–93%. But this distinction is critical: 2 mm is NOT a diagnosis. It is a clue.
